Persons claiming any interest in such funds or any portion of those funds shall file their written claim with the Collier County Clerk of the Circuit Court. Claimant shall make sufficient proof to the Clerk of the Circuit Court of ownership which can be receipts, billings or other information backing up the claim. Claimant must also provide a valid driver’s license or valid government issued photo identification. The affidavit for unclaimed monies BOCC Affidavit or Clerks Affidavit must be completed. If mailing the claim, the affidavit must be notarized and mailed together with a copy of the driver’s license or government issued photo identification and other claim documentation. Businesses must submit their claim form on their business letterhead and the taxpayer identification number must be listed on the form. Unclaimed Monies – Board of County Commissioners: All monies pertaining to the Board of County Commissioners that remain unclaimed after a required dormancy period of one year will be remitted to the Florida Department of Financial Services, Bureau of Unclaimed Property in accordance with Florida Statutes Chapter 717, The Disposition of Unclaimed Property Act. You may search unclaimed monies that have been remitted to the State at their website, www.fltreasurehunt.gov. BOCC Outstanding Checks BOCC Affidavit Unclaimed Monies – Clerk Monies paid on behalf of the Clerk of the Circuit Court remaining unclaimed prior to January 1 of the preceding year will be remitted to the State of Florida as revenue in the Fine and Forfeiture Fund and can no longer be claimed per Florida Statute Chapter 116.
